Around 11:17 a.m., former Prime Minister Shinzo Abe arrived to show his support for a candidate running in the House of Councillors election. He was surrounded by Security Police (SP). Around 11:26 a.m., the SP were ordered to change positions in response to the growing audience in front of Abe. Meanwhile, Yamagami was on the sidewalk behind Abe.

The National Police Agency concluded that the lapse in security behind Abe was caused by "a lack of security reinforcements after notifying security personnel of the change in plans" and "a failure to foresee danger during the planning stage." In order to "never let such an incident take place again, " the NPA vowed in a report to increase NPA involvement in VIP protection and strengthen their security system.
